Integrated health and social care recognises that the health and wellbeing of individuals cannot be addressed by medical or social care systems working in isolation. Mental and physical health, housing, social support, employment, and family are all interconnected, and the most effective care is delivered by professionals who can think across those boundaries and work collaboratively with colleagues from different backgrounds and disciplines.
